Please provide the following items to the main office: 

1.  Original Birth Certificate OR Passport  OR  STATE ID. 

2. Doctor's note that states you are physically fit for "work" (Sports Medicals, blood work are NOT accepted as proof). 

3. Parts 1 & 2 of the AT-17 Form Completed
